Dr. Dariusz Nasiek presented on a topic near and dear to his heart: Decoding Complex Cases: Exploring Severe Low Back Pain Despite Negative MRI & EMG Findings. Dr. Dariusz Nasiek, a triple board-certified physician, possesses extensive knowledge and expertise in pain management. So, what happens when a patient complains of low back pain but the MRI and EMG are negative? Is the patient exaggerating, or is there an overlooked spine condition? Pain is not visible—it cannot be seen on X-rays or MRIs. It’s a physiological phenomenon that requires specialized diagnostic techniques. The differential diagnosis of such complex cases begins with a Lumbar Medial Branch Block (MBB). This diagnostic tool is used to identify facet joint pain. A positive response to MBB with temporary pain relief confirms facet arthropathy and is the gold standard for diagnosing complex pain conditions. It requires two independent diagnostic blocks with more than 50% pain relief, increased range of motion (ROM), and decreased muscle spasms. The gold standard represents the highest recognition in evaluating complex conditions in medicine.
Following a proper diagnostic regimen, therapeutic approaches to relieve pain include Radiofrequency, Radiofrequency Ablation, and Rhizotomy of Medial Branches. Radiofrequency Ablation provides long-term relief through nerve denervation, effectively reducing pain and improving function. Endoscopic Facet Ablation is an advanced technique for precise visualization and ablation. This procedure has demonstrated success in providing extended pain relief compared to traditional methods.
